Patient Advocate & Geriatric Care Coordinator
Older adults and their families are too often left to navigate a healthcare system that wasn't built for them — juggling specialists, discharge paperwork, insurance denials, and decisions no one prepared them for. SageAlly exists to walk alongside them. We're a geriatric care management and healthcare navigation practice, and we're looking for our founding Patient Advocate & Care Coordinator to help us do this work with skill and heart. If you've spent years helping older adults move through the system and wished you could do it the right way — with time, attention, and real advocacy — this is that opportunity. ABOUT THE ROLE Part-time and flexible to start, with room to grow toward 20–40 hours/week over the coming months as our patient load builds. You'll work remotely, but you need to know the New Jersey landscape well — the providers, the programs, and how it all fits together. You'll be the person a family calls when they don't know who else to call. WHAT YOU'LL DO
- Coordinate care for older adults across providers, settings, and transitions (hospital-to-home, facility placement, changes in care level)
- Advocate for patients and families — in appointments, with insurers, and across the system
- Help clients understand their options and make informed decisions, without the overwhelm
- Reach out to physician offices, hospitals, insurers, and community organizations on a client's behalf, following each client's needs to get them the answers and care they're after
- Support families with education, guidance, and a steady presence during stressful moments
- Keep clear, timely documentation — and help us refine the tools we use to do it
WHAT WE'RE LOOKING FOR You're experienced, resourceful, and genuinely good with people under pressure. Must-haves:
- Hands-on experience coordinating care or advocating for older adults — ideally from a hospital, dialysis, home-care, hospice, or geriatric setting
- A solid working knowledge of how the New Jersey healthcare system works — Medicare, Medicaid, and how to actually get things done with local providers and programs
- Available during some weekday business hours to reach providers' offices (hours start light and flexible, and grow over time)
- Tech-comfortable: able to work in an EHR and Google Workspace, and willing to learn and help shape new tools as we build them
- A strong working understanding of HIPAA and patient privacy
- Calm, warm, and professional with distressed patients and families — and driven by the work itself, not just the paycheck
Preferred (not required):
- A license or certification — LPN, RN, LSW, or a credential like BCPA or CCM
- Experience with Chronic Care Management (CCM) and related tools

- Engagement: Independent contractor (1099)
- Schedule: Part-time / as-needed to start; potential to grow to 20–40 hrs/week
- Location: Remote — must be based in, or deeply experienced with, New Jersey
- Rate: $40/hour
- Growth: A founding role with room to take on more as we expand our services and patient base
